Introduced and coordinated by ISBEM, the project intends to detect and to experiment effective and prosecutable solutions concerning the different problems that lead to the formation and to the increase of waiting lists, with the purpose to rationalize the link between services demand and supply and to reduce the discomfort experienced by users in the healthcare services.

The aim of the project is to identify and to experiment measures meant to rationalize the link between services demand and supply, to reduce the discomfort experienced by users in the healthcare services.

The project intends to detect and to experiment effective and prosecutable solutions concerning the different problems that lead to the formation and to the increase of waiting lists, with the purpose to guarantee to all citizens specific times for the access to the healthcare services and, above all, times suitable for their clinical problem. The actions convergent to the attainment of such objective consist in:

  • Definition of an informative system model for the monitoring of the waiting times;

  • Interventions to verify and to improve the appropriateness of the demand;

  • Activation of organizational analysis procedures for resource reallocation and internal reorganization;

  • Activation of systems aimed at stimulating MMG and medical specialists, so as to improve the access to the healthcare services;

  • Identification of the demand of healthcare services through the information drawn by analyzing the waiting times;

The project foresees the constitution of an operational Committee, formed by the Scientific Coordinator of the project and by the Scientific Responsibles of the different operational unities. Initially, the attainment of the objectives will involve a cognitive investigation aimed at verifying the existence of an informative system able to monitor the waiting lists and subsequently, whereas present, the analysis of its efficiency. On the base of the results drawn, it will be created an appropriate informative system model in order to support the regional decision maker.

At the same time, particular tools will be experimented in order to increase the clinical appropriateness of the performances through the sharing and the adoption of diagnostic therapeutic guidelines and/or protocols achieved in accordance to valid and collective methodological runs. Besides, as the role plaid by general medicine in the government of healthcare demand has been generally accepted, the budget institute will be studied as a tool usable to actively involve these professionals in the management of the access to the healthcare performances.

From the point of view of the offer it will be necessary to identify the intervention priority areas  in which to experiment the reorganization of the structural, human and technological resources, thus maximizing its use.

Another experimental area of the research project will focus the attention on the definition of the clinical priorities in the management of the waiting lists, by combining the performances into homogeneous waiting groups on the basis of the gravity of the patient, with the purpose not to jeopardize his prognosis.

The identification of a minimum set of indicators will allow to certify the effectiveness of the interventions undertaken, by monitoring the waiting times both at a macro level (Regions) and at a micro level (Local Health Units and Hospitals).

Finally, through ad hoc investigations, we will try to evaluate the elements that more than others engrave on the satisfaction of the citizen and therefore on the perception of the quality of the national Healthcare Service.

The expected results are divided into two different typologies: methodological documents and products. The first ones are: 

  • Review of the significant experiences at a national and at an international level;
  • Creation of a common glossary concerning the waiting lists;
  • Detection of information and indicators necessary for the supervision and for the management of the access to healthcare performances;
  • Supply and service modalities of the waiting lists’ management systems;
  • Suggestion for the creation of systems specific for the publication of information concerning the waiting times;
  • Arrangement of a form for the professional updating of GPs on managerial topics;

The products are:

  • Report on the monitoring of waiting times relative to selected performances in the regions that take part to the project;
  • Algorithm for the management optimization of structural, technological and professional resources pertaining outpatient performances;
  • Report on the variability determinants in the access to outpatient performances;
  • Report on the criteria necessary in order to prioritize, within the waiting lists, specific people who need tracing performances;
  • Report on the forms for the detection of outpatient performances’ demand starting from the information on waiting times;
  • Report on the criteria for the improvement of the suitability of selected outpatient performances;
  • Report on the use of stimulating systems for the promotion of the appropriate use of outpatient performances as far as GPs and specialists are concerned;
  • Report on the consumers’ satisfaction level as to different organizational and managerial solutions of the waiting lists;
  • Document on the procedures that must be performed at a company level, so as to guarantee both the form and the content necessary to feed the monitoring system;
  • Document on the definition of standard report techniques for the monitoring of consumes;
  • Creation of a programming and control system for the district activities (district budget);
  • Development of a report system consistent with the district budget (budget forms for GPs and district budget forms).
The scientific results will be shown at national and international congresses and will be published on famous international journals.
