Introduced and coordinated by the ASSR, its intent is to realize a system for the analysis and the control of the efficiency, inexpensiveness and impartiality of the National Healthcare Service, through the monitoring of the expenses.

The project intends to realize a system for the analysis and the control of the efficiency, inexpensiveness and impartiality of the National Healthcare Service, through the monitoring of the expenses.

In particular, the project proposes to analyse the process of expense development, suggesting:

  • Levels of expense aggregation, with reference to the supplied assistance levels;
  • Methods of data validation and control, with particular reference to the aspects of information significance and timeliness;
  • Standardization of the expense aggregation criteria in order to render it comparable on a national, regional and local base;
  • Criteria of rationalization and development of the systems of adopted informative  pointers and flows, so as to avoid periphery duplications and superimpositions of demands, that are translated in improper burdens, paying particular attention to the determination of the detail levels according to institutional user sectors;

  • Data Communication and Reporting Systems in order to activate useful feedback from the analysis places towards the regions and towards the service suppliers and the customers of the National Healthcare Service;

  • Use perspective for the direction (forecast and simulation), the control (benchmarking and systems of efficiency and effectiveness pointers) and the research.  

The project foresees the constitution of an operating Committee, formed of the Project Scientific Coordinator and of the Scientific Responsible of the various Operative Units. The intent is to proceed, with the recognition of all the existing informative sources on the healthcare expenses and with the predisposition of a relationship on the possibility to make comparisons among the information deriving from the economic-patrimonial accounting adopted in the various regions.

The monitoring of the healthcare expenses, aiming at the direction and at the control, demands the ability to carry out a standardization of the expenses in comparison to the main variables that influences it, such as:

  • the age;

  • the social-economic characteristics of the population;

  • the geographic and productive characteristics of the territory;

  • epidemiological peculiarities;

  • qualitative/quantitative characteristic of the offer structure;

  • qualitative/quantitative characteristic of the services supplied;

  • factors connected to the institutional organization of the suppliers.

In the first place, it is therefore necessary to estimate, with statistical models, the significance of such expense conditioning factors, consequently identifying standardization criteria, that render it comparable on a national, regional and/or local base.

The second step foresees the drawing of data from controlling techniques, in order to define efficiency and equity pointers (on standardized data) useful for benchmarking.

Furthermore, the project foresees to use techniques of surveying, such as questionnaires to hand out to qualified witnesses, so as to refine the benchmarking system and rendering it adherent to the requirements of the regional local government. In order to estimate the possibility of being implemented of the monitoring system set up in the previous phases, the most suitable tool is that of a sample survey, aiming at highlighting the possible local informative needs, that can be usefully delegated at a central level, thus stimulating the participation of  the local health units to the monitoring system.

The objective consists in the validation of a particular computer device so as to homogenize and to translate all the bookkeeping and extra bookkeeping information coming from health units that have adopted different economic-patrimonial accounting models.

As far as the definition of a healthcare expenses prevision model is concerned, it will be necessary to carry out a careful survey of the economic and medical-managerial literature, in order to collect and to test the prevision and simulation techniques of the expenses in different economic and social-demographic contexts.

The attended results of the project are distinguished in two different typologies: methodological documents and products. The first ones are:

  • A Methodological Document for the calculation of pointers;

  • Criteria of standardization of the expenses;

  • Techniques for the benchmarking.

The products are:

  • Detection of methods for the activity of assessment and control of the expense;

  • Methodology for the forecast of the healthcare expenses at a regional and national level;

  • Computer device to homogenize the bookkeeping data.

The scientific results obtained will be it introduced at national and international conferences and will be published on international reviews of prestige.
